The U.S. Department of Labor’s Occupational Safety and Health Administration has cited a Butler County company for violations that resulted in an electrocution fatality.
Harmony-based Insight Pipe Contracting LLC was notified by OSHA that they are facing over $330,000 in penalties as a result of the incident in April at a worksite in Johnstown, PA.
OSHA began an investigation after an Insight employee was fatally electrocuted and two others who attempted to assist him needed to be hospitalized while they were making a trenchless sewer repair.
The company has been placed in OSHA’s Severe Violator Enforcement Program for violations including training employees on confined space hazards, use of a retrieval line, and completing the proper permits.
Insight has 15 business days after receiving the citations to comply, request an informal conference with the local OSHA administrator, or contest the findings before an independent commission.
